What is a First Report of Injury Form?

If you suffer an injury at work, the first thing you must do is seek the appropriate medical attention. Next, you will want to make sure you inform your employer of the accident for them to make appropriate documentation.

First Report of Injury

When informed of an on-the-job injury, either you, your doctor, or your employer will complete a form called a First Report of Injury [“FROI”] and submit it to the Ohio Bureau of Workers’ Compensation (BWC) or, if your employer is self-insured, your employer’s human resource staff who deals with workers’ compensation.

The FROI will initiate your workers' compensation claim. Whoever completes the form should provide as much detailed information as possible, including:

  • Description of Accident: A step-by-step account of exactly how the injury occurred.
  • Body Parts Affected: Specific details on what was hurt (e.g., "lower right back" rather than just "back").
  • Witness Information: Names and contact info of anyone who saw the event.
  • Medical Treatment: Details on where you sought initial care.

Important Filing Deadlines

Currently, Ohio law gives injured workers one year from the date of injury to file their claim with the Bureau of Workers’ Compensation.

Previously, the statute of limitations for occupational diseases was longer. However, under House Bill 75, which became effective in September 2021, that rule changed.

It is critical to keep these current deadlines in mind: * Traumatic Injury: Claims must be filed within 1 year of the date of injury. * Occupational Disease: Claims must be filed within 1 year of the disability due to the disease or the diagnosis of the disease (whichever is later).
